Family Rate is available when 2 or more family members enroll
and maintain active memberships - Rate as listed is per person.
One time Registration Fee of $100 per person. Registration fee not to exceed $200 per family.
Registration fee increases by an additional $100 with every re-enrollment.
(“Re-enrollment” occurs if a member decides to cancel their Lake Life DPC Membership, then re-enroll at a later date.)
Members will be charged on the 1st of the month for the following month.
